Making Changes to your Account

 

The Service Change Order Form

Changes can be made to your account using the Service Change Order form which you receive with your shipment of badges as part of your packing list. Deletions are made by clearly printing a "D" in the column provided on the front of the form; additions and reactivations by using the grid on the back of the form. Other changes can be made using the same form, which carries its own instructions.

 

Faxing your Changes

You may fax us your changes using the form available from Landauer. Please e-mail us now to request a copy.

Please remember to quote your account number and series code, if applicable, on the form.

Please note that your account number, series code if applicable, names and participant numbers of the relevant individuals must be quoted in all correspondence.
