The Radiation Dosimetry Report

 

At the end of each exposure period, when dosimeters are returned to us, a report will be issued showing the type and amount of radiation exposure received by each participant. Explanation and remarks can be found on the reverse side of each report. Explanations of some key items are given below.

1. Report Heading

  1. Account No: This is the number permanently assigned to your account.
  2. Series Code: Within a particular account, you may wish to distribute dosimeters to various sections or departments. The series code identifies these departments and allows us to print separate reports for each series.
  3. Process No: A number assigned to a group of returned dosimeters which enables us to follow all production operations through to the actual report print-out.
  4. Report Date: The date on which the report was printed.
  5. Quality Control Release: The initials of the person ultimately responsible for approving the report before it is sent out.

 

2. Report Body

  1. Participant ID Number: Within your account, each individual is identified by a permanent five-digit number. This number applies to all activities concerning the individual, even if he or she has more than one badge. It should always be quoted when requesting information or changes affecting that participant.
  2. Participant’s Name: This normally shows the name of the participant, but can also be left blank, assigned as a spare, area monitor or other designation, depending on the use and accompanying name you have given a badge.
  3. Notes: This column gives coded information about individual dosimeters. E.g. B-2 means that a dosimeter may have been damaged by moisture.
  4. Dosimeter Type: Indicates the type of dosimeter used.
  5. Use: Indicates the part of the body a dosimeter was used to monitor. E.g. 1 means "whole body."
  6. Radiation Quality: Radiation Type, and in some cases energy. E.g. PH means "Photon - High Energy."
  7. Exposure to dosimeter: Shows the current exposure for deep and shallow dose equivalents.
  8. Cumulative Totals: Show the accumulated exposures for deep and shallow dose equivalents.
  9. Adjustments.
  10. Sex, Birth Date and National Insurance Number: In the case of a classified account, these details are shown.
  11. Number of dosimeter reports: Shows the number of reports made for an individual in the current quarter and to date.
  12. Inception date of permanent total: Month and year of the beginning of continuous Landauer service for a given participant.

 

Whenever requesting information about a specific report, always quote your account number, series code if applicable, report process number and, if querying an individual’s dose, the participant number of the individual in question.
