Spare Badge Assignment Sheet

 

Assigning a spare badge means that the measured exposure on the spare dosimeter will be assigned to the dose record of the individual that wore it.

The individual may be an existing badge wearer using a spare as a temporary replacement or additional badge, a one-off user (a visiting worker for example) or a new user who will require normal monitoring in the future.

Spare forms are available from customer services. To receive one, e-mail us now.

1. To assign a dose to an existing participant:

a. Complete columns 1-3 using the information on the spare dosimeter.

b. Tick the column headed Regular Badge User.

c. Complete columns 9-15. Please note that this information refers to the individual wearing the badge and has nothing to do with the spare itself.

2. To assign a dose to a new participant

a. Complete columns 1-3 using the information on the spare dosimeter.

b. Tick the column headed New Badge Required After This One

c. Complete columns 7-15, omitting column 7 unless you have previously requested service for the individual and already know his or her assigned participant number. This information must be relevant to the new wearer and his or her requirements and not to the spare badge itself.

3. To assign a dose to a one-off user

a. Complete columns 1-3 using the information on the spare dosimeter.

b. Tick in the column headed one-off use

c. Fill in the other columns as appropriate

 

Please note that your account number, series code if applicable, names and participant numbers of the relevant individuals must be quoted in all correspondence.
